VOS JUNETEENTH VENDOR AGREEMENT
REQUIRED DOCUMENTS
General Liability Insurance All Food Vendors are required to have general liability insurance. Please communicate these instructions to your insurance company: List the VOS JUNETEENTH Celebration and the City of Phoenix as additional insured. INSURANCE IS REQUIRED, REGARDLESS OF PROCEDURES THAT MAY HAVE BEEN FOLLOWED AT PAST EVENTS.
We will not consider your application until it is complete. Space is reserved on a first come, first served basis as determined by submission of all paperwork and fees. Completion of application does not guarantee acceptance into the event. There is a $25.00 NSF fee for returned checks. The VOS JUNETEENTH Celebration is a rain or shine event. Vendor fees will not be refunded.
EQUIPMENT
The fee is for space, table, chairs and tents. There is a $50.00 charge for each electrical connection.
All pertinent fire codes, laws, ordinances, and regulations pertaining to health, fire prevention and public safety shall be strictly observed. The VOS JUNETEENTH Committee and Eastlake Park and Community Center will not be liable if any government representative closes a booth that does not comply with regulations. Vendors closed by an inspector will not receive a refund.
NO STAKING
No staking will be allowed on the campus unless express written permission has been granted in advance by VOS Juneteenth Committee or Eastlake Park officials.
SET UP/TEAR DOWN - VENDOR
Set up: 1:00 pm - 4:00 pm on Saturday, June14, 2014, or unless otherwise noted.
Tear down: When feasible by vendor discretion
The VOS JUNETEENTH Committee, agents, employees, volunteers, City of Phoenix and Eastlake Park and Community Center are not responsible for any loss or theft during the run of the Event or set up and tear down.
PARKING
All vendors will receive information for designated vendor parking area.

DEADLINE FOR APPLICATION SUBMISSION IS June 2, 2014.
CANCELLATION POLICY
Cancellation must be submitted in writing and mailed or faxed to Valley of the Sun Juneteenth (602)304-1088 Cancellation on or before June 5, 2014, will result in a forfeiture of 50% of the booth/space rental cost. Cancellation after June 5, 2014, will result in the forfeiture of the entire payment of booth/space rental.
PHOTO RELEASE
The undersigned does hereby give consent to the VOS JUNETEENTH Committee and assignees to take photographs, make audio recordings and/or video graphs of myself and/or my organization at the VOS JUNETEENTH Celebration and to use such photographs, audio recordings and/or video graphs for such promotion, publicity or other lawful purpose as the VOS JUNETEENTH Committee deems appropriate. I/we further waive any rights of claims as to content approval or damages that I/we may have for use of these reproductions.
I have read and agree to abide by the terms and agreement of the 2014 Valley of the Sun Juneteenth Celebration and other government entities as may have jurisdiction. I hereby release and hold harmless the VOS Juneteenth Committee, MCCCD, South Mountain Community College, and their officers, employees and agents from all liability, including, but not limited to, statutes, ordinances, and any legal authority or cause, theft, personal injury, strike, public enemy, or act of God, and do indemnify these named organizations from any damages that may arise from doing business at the 2014 VOS Juneteenth Celebration. Vendor fees are non-refundable. I understand that all vendors are subject to approval by the VOS Juneteenth Committee.
